If there is a yellow message at the bottom of the contact form click on Detail to view the message indicating the error.

In this example the Electoral Division has been set as a default to be populate when creating a contact and this particular record does not have one selected. This could be because the default was not set at the time of creation.

Click the pencil and select the information.

The Lookup Value that sets the Electoral information is under the Contact folder
