You can include information from a user for contacts related to them such as a Spouse, Children or any other members of their household. To do this you will need to add an Additional Contact composition to the form. You can do this as follows:
1. First make sure that you have added a basic contact composition or otherwise the Additional Contact composition will not work correctly.
2. Click on the + Icon below the contact composition you created earlier:

3. Select Additional Contact:

4. Select the + icon to add components to this composition:

| 5. | Many of the components in this page are similar to the ones in the Contact composition and work identically. The only new component here is Relationships which allow you to set the relationship between the additional contact and the new contact: |

| 6. | Add the Basic Info Component (this is required for Additional Contact to be created) and configure it. Then Add the Relationships Component (this is also required): |

7. Configure the label that the user will see when selecting a relationship and then add a relationship:

| 8. | From here you can add each relationship that want your users to choose from. If you only have one option the Relationship selection will be invisible to the end users, if there are two or more the user can select which relationship they want to add. You can add as many relationship choices as you want to the page: |


| 9. | Once you have completed the setup of this additional contact you can also configure a few other settings for this composition including the Label, Apply Household and Minimum/Maximum Quantity. |
10. Label determines the text used for this component that hovers just above the + button:


| 11. | Apply Household adds this contact to the Main Contact's household on the downloading of these records into thankQ. Untick this if you don't want this relationship applied to this contact's household (this setting applies to any of the relationship choices): |

| 12. | Minimum Quantity and Maximum Quantity determines the minimum and maximum number of related contacts a user can add. This can be useful to require at least one related contact and it can also be used to limit the number of additional contacts per user: |

13. When you are finished select Save and publish.
