The Profile tab that is created will be visible on the Contact Details form as an additional Profile tab

Setup all the Profiles required in Lookup Values under Contact.

Profile – Multiple is where you would enter any information that can be used more than once eg can have several answers.

Profile – Single is used when there is only one answer and cannot be used more than once eg Age Range
