1.If you wish to have a heading or question before the check boxes, enter this in the Name Fields Additional Info Grouping Label field.

2.Then enter the options in the Name Checkbox Label fields.  You can have up to 10 options.

3.To see how your name heading and check box fields appear, click Save and then Preview.
