Setup the relevant Single and/or Multiple Profiles that will be used in the User Defined tabs on the contact records.

Single Profiles are used where there can be only one answer

Multiple Profiles are used where there can be many answers

Once the Profiles required are setup they can be used in User Defined Tabs
